The Telluride Academy has new forms for 2010:
The forms are available in a single editable PDF file. You may download the PDF file, fill in the forms on your computer, print them out, sign them and return them to us. Please review each of the three required forms carefully and note that there is a space for both parents and student to sign. There are three forms, each of which is two pages long.

Completed forms (a total of 6 pages) are required for each student.

Please note that registration will not be considered complete and your spot cannot be secured until all forms and a 25% deposit have been returned to the Academy in person, by fax or by mail. Final payment is due May 15, 2010.

These forms are a vital way you can get more information about the Telluride Academy, and we are able to learn more about your student - thanks for completing them in a timely manner!

STEP 1:  Complete the online REGISTRATION

STEP 2:  Download the form.  This single PDF file contains 3 separate 2-page forms (Registration & Terms of Agreement, Medical and Health Information Form, and Acknowledgement of Risk).

STEP 3:  You may either print out the blank form and fill in it, or open the file and use your computer to fill in the form fields.  Then PRINT, SIGN, and FAX all 6 pages to 970-728-5312.
