The Telluride Academy has new forms for 2008: Please review each of the three required forms carefully and note that there is a space for both parents and student to sign. Each of the forms is two pages long.
**A copy of all three forms is required for EACH camper.**
Please note that registration will not be considered complete and your spot cannot be secured until all forms and a 25% deposit have been returned to the Academy in person, by fax or by mail. Final payment is due May 15, 2008.
These forms are a vital way you can get more information about the Telluride Academy, and we are able to learn more about your student - thanks for completing them in a timely manner!
